Forms 1095-C
As of 1/31/25, Form 1095-C copies are available by request. Previously large employers were required to send Form 1095-C to all applicable employees by 3/2. Effective for the 2024 reporting year, new legislation has been signed into effect that no longer requires employers to furnish a paper copy to all employees.
To view or receive a copy of your form, please follow the below steps:
For employees that have access to the iVisions employee access portal, copies of your Form 1095-C will be posted to the portal by 3/2. For employees that do not have access to the iVisions employee portal, you may request a copy of your 1095-C from Terryann Allison at tallison@ffc8.org.
As a reminder, Forms 1095-C are for your records, only. The forms provide information about health insurance coverage offered by the district. They are not needed prior to filing your personal income taxes.
